The ZBiotics Feedback Survey (the "survey") is subject to the following terms and conditions:

• ZBiotics may choose to end the survey without notice at any time.
• Limit 1 response per ZBiotics customer.
• Limit 1 response per ZBiotics purchase.
• Response reward is contingent on verification of purchase.
• Response reward is contingent on successful completion of all mandatory questions in the survey.
• ZBiotics may change the survey and these terms at any time.
• ZBiotics reserves the right to deny rewards to survey participants if ZBiotics feels there has been abuse of the survey process or other circumstances which make such denial a reasonable course of action.
